New and Revised Judicial Council Forms and Local San Diego Superior Court Forms
Effective July 1, 2007, the Judicial Council of California and the San Diego Superior Court have implemented some new forms, and made revisions and revocations to some existing forms.
The San Diego Superior Court now has the new and revised Judicial Council forms and local San Diego Superior Court forms available for distribution in its business office located in the Hall of Justice at 330 West Broadway. The new and revised forms may be used immediately and will be required by the Superior Court beginning Friday, August 24, 2007. After August 24th, the previous versions of the forms will no longer be available or accepted for filing by the court. Additionally, the Superior Court will no longer customize the Judicial Council forms by assigning local form numbers to them.
Judicial Council revisions of note include the "Civil Case Cover Sheet," form number CM-010; this form is required to be filed with all new civil cases. Judicial Council revocations of note include the forms "Request and Order for Free Service of Restraining Order for Sexual Assault or Stalking," CH-101 and DV-290.
Other revisions include the Judicial Council's California Law Enforcement Telecommunications System (CLETS) Information forms for domestic violence, civil harassment and elder abuse. All three of these have been combined onto one form. The separate form numbers, DV-260 / CH-102 / EA-102, are listed at the top of the new form. The forms are available according to each grouping on the Judicial Council's website, i.e. domestic violence (DV), civil harassment (CH) and elder abuse (EA).
The new and revised Judicial Council forms are available on the Judicial Council's website at www.courtinfo.ca.gov/forms.
The San Diego Superior Court's newly revised forms, "Stipulation to Use of Alternative Dispute Resolution Process" number CIV-359 and the "Pre-Read Request for Hearing" number D-117, are available at their business office or on the Superior Court website at www.sdcourt.ca.gov, click on "Forms" in the menu on the right hand side of the screen.
For further information and a list of the New / Revised / Revoked forms, visit the San Diego Superior Court's website at www.sdcourt.ca.gov and click on the "Announcements, Notices to Attorneys & News Releases" link on the right hand side of the page, or contact the Civil Business Office at (619) 531-3141.
